Why Redaction Matters
Protecting the privacy of your residents and staff is paramount. CareHomesSupportDocs enforces a strict zero-tolerance policy for the publication of Protected Health Information (PHI) and Personally Identifiable Information (PII). Failing to redact documents violates HIPAA and state privacy laws.
What Must Be Redacted (Blacked Out / Removed)
- Resident Identifiers: Names, initials, dates of birth, Social Security Numbers, admission dates, medical record numbers, room numbers, and specific medical diagnoses that could identify a resident.
- Staff Identifiers: Names, initials, home addresses, phone numbers, and employee ID numbers. (Job titles, e.g., "Med-Tech" or "Administrator," are generally acceptable).
- Family Identifiers: Names, contact information, and relationships of resident family members.
- Financial Information: Bank account numbers, payment records, and billing details.
Proper vs. Improper Redaction
- ❌ Improper (Do Not Do This): "Resident John Smith was given his medication late on 10/12/2023."
- ✅ Proper: "Resident #1 (R1) was given their medication late on [REDACTED]."
- ❌ Improper (Do Not Do This): Uploading a PDF and drawing a semi-transparent box over a name. (The text can still be highlighted and copied!)
- ✅ Proper: Physically blacking out the text with a marker and scanning the document, OR using professional PDF redaction software that permanently flattens and deletes the underlying text metadata.
